Bridging The Digital Divide: Where SDOH and Telehealth Collide
It’s our last webinar for the year! This one’s all about evaluating social determinants of health (SDOH) and connecting people to community services and resources that address a variety of issues (food deserts, homelessness, safe and sanitary living environments, access to healthcare services, lack of broadband access, economic stability, etc.) are foundational to individual and collective health.
Our speaker will discuss how analyzing SDOH can be used to inform planning and successful implementation of an equitable Digital Health strategy. Topics include:
• Examining tools available to conduct a community health needs assessment.
• Describing where to locate lists of available resources in target areas.
• Discussing ROI examples for SDOH programs that are powered by telehealth technologies.
